Our Referral Process

Families are welcome to reach out to us at any time to ask questions or learn more about our services. In most cases, the next step is for the individual’s Supports Coordinator (SC) to contact GCS and share the Individual Support Plan (ISP), along with important details such as support needs, staff preferences, preferred days and shift times, and any other considerations that will help us understand how to best support the individual.

Our referral team carefully reviews all submitted information to determine whether we are able to provide services and staff the supports as requested. Once this review is complete, we communicate our determination directly with the Supports Coordinator.

If we are able to accept the referral, a member of our leadership team will connect with the family to begin the initial intake process. From there, we work collaboratively with the family and Supports Coordinator to finalize details and prepare for services to begin. If we are not able to accept the referral at that time, we document the individual’s information and will reach back out to the Supports Coordinator in the future should availability change.

At this time, we provide services for individuals enrolled in the Person and Family Directed (PFD) Waiver, Community Living Waiver (CLW), and Consolidated Waiver.

Initial Inquiry

Families or Supports Coordinators (SCs) may contact GCS to ask questions or learn more about available services.

Referral Submission

The individual’s SC reaches out and provides the Individual Support Plan (ISP), along with relevant details such as support needs, staff preferences, preferred days and shift times, and any important considerations.

Referral Review

Our referral team carefully reviews the information to determine whether we can meet the individual’s needs and staff the requested supports.

Determination & Follow-Up

GCS communicates our determination directly with the Supports Coordinator.

Intake & Service Planning

If accepted, a member of our leadership team connects with the family to begin the intake process and collaborates with the Supports Coordinator and family to prepare for services to begin.

Future Opportunities

If we are unable to accept the referral at that time, the information is documented and we will reach out to the Supports Coordinator if availability changes.
